NOTICE – CCU Medical Plan Change of Provider
Greetings Members,
Please be advised that our medical plan provider has changed. As of December 1st, 2022, The GENMed Group Health Plan will be available at CCU.
Existing members of the CredUMed plan will automatically be transferred to the GENMed Plan.
New Members who wish to join the GENMed Plan will need to complete the Medical Questionnaire, ACH and Enrolment forms which are available at your CCU Office or can be sent to you via email.
