- Job Letter (No older than 6 months)
- Payslip (Most Recent)
- 2 Forms of ID (Valid photo ID)
- Proof of address (No older than 3 months)
- Membership Fee: $10.00

Documents Required for CUNA – FIP:
- Complete Cuna – FIP Application
- Two (2) forms of ID
- Spouse – Marriage Cert
- Children – Birth Cert
- Members over the age of 18 must have one form of ID
