Frequently asked questions
- The plan you select can cover you and any combination of the following persons:Â
- Spouse (common-law included)Â
- Any combination of up to two persons from your parents or parents-in-law. These persons must be enrolled before age 76.
- Up to five (5)Â children (aged 1 through 25 and who are not yet married). Children who are permanently disabled are covered for the duration of their lives once they are enrolled before age 26.Â
You must complete and submit the Enrollment Form to the Credit Union along with two months’ premium payment and all certified copies of all the required documents.
Payments can be facilitated over the counter, via salary deduction, standing order or bank transfers.
The six (6) month waiting period is the period of time during which an Insured Person is only eligible for a Benefit which covers the Insured Person’s death if it occurs as a result of an accident.
The following documents must be submitted:Â
- Death certificate or Proof of Death form.Â
- Police report in cases of accidental death, if available.Â
- Identification for claimant and the deceased person i.e. National Identification, Driver’s Permit or Passport.Â
- Recent Utility Bill.Â
The beneficiary designated by the Member on the Designation of Beneficiary Form, or to the Insured Person’s estate if the Insured Member is the only Insured Person listed on the Member Certificate. Other persons listed on the Member’s Certificate in the following, descending order:Â
- The Insured Member, if living.Â
- The Insured spouse, or significant other, if any.Â
- The Member’s Insured parents (equally), if any.Â
- The Insured parents (equally) of the Insured Member’s spouse or significant other, if any.Â
- One parent of the Insured Member and one parent of the Insured Member’s spouse or significant other, if any.Â
- The Insured unmarried, dependent children and the Insured unmarried, dependent children under legal guardianship of the Member (equally), if any, or their legal guardian (held in trust for minor children).Â
- The Insured Member’s estate.Â
Once enrolled you may not choose a Plan with lower benefits. However, if you wish to reduce your coverage you must cancel your existing policy, re-enroll in the new, lower-benefit Plan and be subject to the six (6) month waiting period. You can upgrade your Plan only after the initial six (6) month waiting period has elapsed and no more than once every 12 months thereafter. There will be a six (6) month waiting period for the higher benefit. If a claim is submitted during the waiting period the benefit will be based on the original Plan except in the case of accidental death.
