Self-employed | CCU Membership Application
Description: For individuals operating their own businesses.

- Certificate of registration.
- Notice of Address of the Business.
- Sole Trader statement.

Retiree | CCU Membership Application
Description: Membership for retirees receiving pensions.

Two (2) forms of valid photo identification (e.g., National ID Card, Driver’s Permit, Passport).

Birth Certificate and Picture Identification of the minor.
